Dapoxetine

Dapoxetine is a selective serotonin reuptake inhibitor developed specifically for premature ejaculation rather than for depression. The European product information sets out an unusual dosing pattern for an SSRI, taken before an occasion rather than every day, and one safety point that follows from it.

Key points

  • Priligy is used to treat premature ejaculation in men between 18 and 64 years old.
  • Dapoxetine belongs to the group of medicines called selective serotonin reuptake inhibitors, and Priligy may also be known as a urological medicine.
  • It is taken 1 to 3 hours before sexual activity, and not more than once every 24 hours.
  • The usual dose is 30 mg, and a doctor may increase it to 60 mg.

How it works

Dapoxetine is an SSRI, the same class used for depression, but its handling in the body suits use on demand rather than daily. That is why the product information instructs taking it only 1 to 3 hours before sexual activity is anticipated, and never more than once every 24 hours or every day.

Because it is an SSRI, it interacts with other medicines acting on serotonin, and the product information sets out washout periods in both directions: 14 days after stopping one of those medicines before dapoxetine can be started, and 7 days after stopping dapoxetine before one of them is taken.

Safety specific to dapoxetine

Fainting and low blood pressure. The product information warns that dapoxetine may cause fainting or dizziness, and that patients may feel sleepy, dizzy, faint, have difficulty concentrating and experience blurred vision while taking it. Before starting, a doctor checks that blood pressure does not drop too much on standing up from lying down.

The advice given for a fainting episode is practical rather than abstract: measures that stop you falling and hurting yourself matter as much as the faint itself.

Alcohol. The product information is direct: avoid alcohol when taking Priligy. It notes an increased risk of injury from fainting, or from other side effects, if dapoxetine is taken with alcohol.

Liver problems. It should not be taken by anyone with moderate or severe liver problems.

Stopping it. Dizziness may occur after stopping the medicine, even in someone who has not taken it every day. That is worth knowing in advance for a medicine designed for occasional use, because the effect is not what an as-needed tablet leads people to expect.

When to seek urgent care

Fainting, or feeling faint with dizziness and blurred vision, warrants medical attention and a review of whether the medicine suits you, particularly if alcohol was involved.
